Illinois Public Health Association community partners are conducting Rapid Community Assessments to identify some of the greatest obstacles in access to health care. All responses are kept confidential, and any identifying information is kept separate from the health information. Though the primary focus is COVID-19 prevention, this information is also critical for other community outreach efforts. All responses are appreciated and will be used to better understand the ever-changing health care landscape.
